3. Customer responsibilities

You must not contaminate or substitute samples.

You must not tamper with sample packaging.

4. Clinic collection requirements

Bring acceptable identification when required.

Chain-of-custody testing requires photographic identification.

5. Home collection requirements

Opened biological collection kits may be non-returnable.

Faulty kits will be replaced without charge.

4. Clinic appointments
